Health Records Policy

All student health records will be kept for a maximum of five years after the student graduates or withdraws from RISD.

A physical exam and record of immunizations, required by State law, must be on file at Health Services. These records are confidential and information in them generally cannot be released without the student’s written consent, except when required by law. It is extremely important that you complete all Health Service forms and requirements before classes begin. If you do not supply Health Services with the required health forms, you will be charged a late fee and you may be withdrawn from RISD.
