Student Health Records Policy

Purpose

The objective of this policy is to define who must submit all required medical forms and proof of vaccinations in order to attend RISD.

Scope

This policy affects all registered RISD students, whether physically present on campus or studying elsewhere.

Policy Statement

All matriculated RISD students, both undergraduate and graduate, and Pre-College students are required to comply with Health Services health forms, screenings, and vaccination requirements in order to use RISD Health Services.

Definitions

Electronic Health Record (“EHR”) -- an electronic version of a patient’s medical history that is maintained by the provider over time, and may include all of the key administrative clinical data relevant to that person’s care under a particular provider.

Policy

Students enrolled in at least one RISD course are required to complete and submit Student Health Services health forms, screenings, and proof of vaccination requirements. With the exception of the summer Pre-College program, students taking courses through Continuing Education do not need to comply with Health Services requirements and do not have access to medical care at RISD Health Services. These requirements are based on the Rhode Island Department of Health guidelines and are subject to change at the discretion of the RISD President and Cabinet and as required by changes in law.

All student health records will be maintained and secured from a student’s first year through graduation or withdrawal from RISD. Pre-College students’ records will be maintained and secured during the time the student is on the RISD campus only.

Students who do not comply with the requirements of this policy may not continue to live in campus housing or attend RISD classes, and may subsequently be withdrawn from RISD.

Procedures

Students must submit all required documentation to Health Services through the secure Medical EHR patient portal. Health Services clinical staff verifies the information provided and ensures compliance with federal, state, and RISD requirements. Students are then able to access healthcare through RISD Health Services.
